PIERCING MINORS

To pierce a minor in the state of Oklahoma we require the following information. We require the Childs Birth Certificate and the mothers or fathers' driver's license or state ID. As long as the parents name matches perfectly to the child's birth certificate then we are able to do the piercing procedure no genital or nipple piercings on minors. In some cases, the mothers name may not match the child's birth certificate due marriage, divorce, or remarriage. If that is the case then the mother will need to bring her marriage license(s) or divorce decrees showing from the name she was to the name she is now we have to turn in a paper trail for health department to prove that the parent is the child's guardian.
